According to a survey conducted by the California Healthcare Foundation, 82 percent of Californians say it is important to have end-of-life wishes in writing; however, only 23 percent have done so.

Hospice of the Valley, a leading provider in advancing palliative, hospice care, community grief support and education in Northern California, hosts its Community Life Planning for Your Future: Advance Care Planning Workshop, featuring Karen Storey, a certified master trainer for Respecting Choices, on Thursday, June 12 from 2:00-4:00 pm or 6:00-8:00 pm at Hospice of the Valley, 4850 Union Avenue, San Jose 95124. 

The workshop will help you learn:

• How to start the conversation with loved ones
• How to select the most effective health care agent
• The Do’s and Don’ts of completing an Advance Directive form

An advance health care directive lets your physician, family and friends know what your health care preferences are which includes the special treatment you want or don't want at the end of life, as well as your desire for diagnostic testing, surgical procedures, CPR and organ donation.

The workshop is free. Donations to Hospice of the Valley are gratefully accepted.
